Disability and Accessibility Support Services (DASS) is bound by law and University policy to protect the privacy of each student. Unless required by law, DASS cannot reveal students' names or any other information about them. In the course of work with each student, however, it is sometimes helpful to discuss a student's situation with an instructor, institutional equity representative, rehabilitation counselor, medical specialist, or other professional. Please indicate below if you give your permission to DASS to refer to your name or other information about you, but only for the purpose of providing services to you.
By clicking yes, I authorize Disability and Accessibility Support Services at UM-Flint to communicate with UM-Flint faculty and staff in order to obtain and release information (written or verbal) regarding my disability and its impact on major life activities, particularly those which are academic in nature, as necessary to determine reasonable accommodations. I understand that I may revoke consent at any time and that this revocation must be delivered to Disability and Accessibility Support Services in writing (through email or hand delivered in person).
